WHAT IS A GERIATRIC CARE MANAGER?

A Geriatric Care Manager is a professional who specializes in assisting seniors in meeting their day-to-day and long term care needs. Geriatric Care Managers provide Needs and Problem Assessments, recommend solutions, develop, implement and monitor an appropriate care plan.

In addition, Geriatric Care Mangers screen, arrange and monitor in-home help; arrange for medical, legal and financial services; evaluate living arrangement, housing options and assist with relocating seniors to the most appropriate setting.

Geriatric Care Managers act as liaisons to families who are separated by long distances from their elderly loved ones making sure they are managing well. And, alert the family to any concerns or problems that may arise.

Geriatric Care Managers also offer counseling and support including family conflict mediation and crisis intervention.

Foremost, the Geriatric Care Manager is the client’s advocate ensuring they live with the greatest degree of independence, safety, dignity and comfort.


THE CARE MANAGEMENT PROCESS
